• Bagi Badan Usaha yang ingin membuat sertifikasi badan usaha sebagai berikut :
  
       -  Company profile, Fotocopy akte pendirian, Neraca 1 Tahun terakhir, Fotocopy NPWP
  
       -  Fotocopy SKA/SKT (Arsitektural, Sipil, Mekanikal, Tata Lingkungan)
           sesuai dengan bidang yang di ambil 
  
       -  Fotocopy ISO 9001:2008 (khusus Gred 7 dan PMA) 
  
       -  Berkas-berkas yang harus di lampirkan pada saat permohononan ke asosiasi
          gapeksindo, format dalam microsoft excel, dan word [download]
